Disclosure Statement
The following disclosures apply to your participation in Solo Health Collective, a self-funded health plan operated by Healthy Business Group. More detailed terms are provided in the plan documents you review and sign at enrollment, including the Summary Plan Description, Joinder Agreement, and Medical Deductible Reimbursement Policy.
How claims are paid
Solo Health Collective is a self-funded health plan. Solo does not make payments directly to medical providers. Claims are administered by Vault Admin Services (VAS), Solo's third-party administrator, which processes and pays claims on behalf of the plan. Members are responsible for any applicable cost-sharing amounts - including deductible responsibility - as set forth in the plan documents.

Cancellation policy
Your Solo health plan may be cancelled at any time. To cancel, you must submit a written cancellation notice to support@hbgnow.com at least five (5) business days prior to your next scheduled payment date. Cancellations are not retroactive. Failure to submit written notice will result in continued monthly drafts until written confirmation of cancellation has been received.
Plans renew automatically on January 1st each year. If you do not wish to renew, a written notice of non-renewal must be received prior to that date.
Refund policy
It is your responsibility to cancel your plan on time and in writing. Payments already processed are not refunded. Cancellation requests received after a payment has been drafted will take effect prospectively - no retroactive refunds will be issued for late cancellation requests.
If you believe you have been incorrectly charged, please contact us within five (5) business days of the draft date at support@hbgnow.com or 646-328-6968. Refund requests will be evaluated based on the timing and documentation of the cancellation request.
